SNAP helps low-income individuals and families buy the food they need for good health.

You apply for benefits by completing a State application form. Benefits are provided on an electronic card that is used like an ATM card and accepted at most grocery stores. East Valley Family Services can help your family apply for benefits in this program provided by the USDA Food and Nutrition Service.

  4. ___ Income (most current pay stubs for the last 60 days)

  5. ___ Disability, SSI, or Social Security (current award letter/s)

  6. ___ Letter from any person/s who are helping (paying bills, rent, or

  7.           giving you money)

  8. ___ Bank Statement (most current)

  9. ___ Car(s) Registration(s)

  10. ___Residency Verification (Current Signed Lease/ or letter of

  11.           agreement from landlord.)

  12. ___ All Utility Bills (most current power, gas, telephone or cell etc.)
